MCV4 Vaccine This letter is to inform you that all students entering the 7th grade will need a Tdap vaccine and one dose of Meningococcal (MCV4) vaccine prior to entry.  All students entering the 12th grade will need two doses of Meningococcal (MCV4) vaccine prior to entry.  If the first dose of MCV4 is administered on or after the 16th birthday, a second dose is not required.  The Meningococcal (MCV4) is new this school year.

The one dose of Meningococcal (MCV4) is new this school year. These immunizations are required by Law under the Ohio Revised Code section 3313.67 and 3313.671 for school attendance. These immunizations can be done by your family physician or at the Logan County Health District. The Logan County Health District will be holding a special clinic on Tuesday May 24th from 4:00pm – 6:00pm, please see flier below. The Logan County Health district holds immunization clinics every Thursday from 9:00am – 11:00am, and the first Thursday of each month from 2:00pm – 6:00pm. You may call (937) 651-6186 for an appointment. Please make sure that these are completed PRIOR to The First Day of School, August 17, 2016 and a copy is on file with your student’s school.
