I. DIRECTORY INFORMATION
Directory information concerning a student may be disclosed by the school unless the parent or student eighteen (18) years of age or older has requested in writing that all or part of this
information is not to be released. Such requests shall be submitted within two (2) weeks of initial enrollment into the
Bellefontaine City School District during the two (2) weeks prior to the first day of school of each school year or
within two (2) weeks of a student becoming eighteen (18) years of age. In emergency situations requests to
prohibit the release of directory information will be honored as soon as it is reasonably convenient to do so.
Parents and students will be informed annually of the school
district’s definition of directory information. Procedures for disclosure and for requesting nondisclosure will be
included along with all other rights.

 Directory information records may include the following information:
a. Name
b. Address
c. Telephone Number
d. Date and Place of Birth
e. Participation in Officially
Recognized Activities and Sports
f. Weight and Height of Members of Athletic Teams
g. Dates of Attendance
h. Awards Received
i. Date of Graduation

II. THE STUDENT SERVICES DEPARTMENT
The Student Services Department provides services for students from birth to age twenty-one (21) who have special needs. These services include:
• Special Education programs and related services for disabled preschoolers and school-age children;
• English As A Second Language (ESL) tutoring for students who speak minimal English;
• Chemical abuse prevention and referral programs;
• District-wide crisis intervention;
• K-12 school psychological and guidance counseling services;
• Gifted-Talented Enrichment
programming screening services;
• STATE-MANDATED VISION AND HEARING SCREENINGS FOR STUDENTS IN GRADES K 1 3 5
7 AND 9 AND PART-TIME NURSE HEALTH TECHNICIANS IN EACH BUILDING;
• Student enrollment-attendance services;
• Developmental screening for preschoolers enrolling in kindergarten;
• ADMINISTRATION OF STATEMANDATED
TESTING IN GRADES 3 TO 8 AND 10 AND
DIAGNOSTIC TESTING IN GRADES K TO 2.
